What is Complex Care?
Complex Care means the level of care that we provide for individuals with multiple health and / or social care needs, often requiring clinical support. This could include those with chronic or long-term health conditions, life-limiting illnesses or physical disabilities.

Building your care team
We recruit and build care teams specifically for each family that we work with in mind.
It’s really important to us that they are going to be a good fit for our client.
Aspects such as location and previous experience and are taken into account, and although this extra work we put in can take time, we believe it provides a better chance of a successful and sustainable, long-term package of care.
We are aware that introducing new faces into your family home can be challenging, so we arrange ‘meet & greet’ visits to allow everybody to get know each other and ask any questions before we start working in your home.
Clinical Support
This creates a consistent team that will get to know you, your family and your care team and join you on your journey together.
The Lead Nurse will oversee your care package, attend any progress meetings, review care plans and keep in touch with your family, professionals and staff to ensure everything is safe and that everybody is happy.
